1. Current 5 years of loss runs from your current insurance company. Request for Loss Runs (Download)
2. A copy of your current budget and balance sheet including reserves
3. Association documents (By-Laws/Covenants)
4. Declaration pages for your current policies or a certificate of insurance showing your current limits and deductibles
5. Building Improvements (major repair or replacement) of each of the following:
- Roofing Year
- Electrical Year
- Plumbing Year
- HVAC Year
6. A statement as to how many rentals there are
7. Board President's:
- Name
- Phone#
- Email
8. FEIN (Federal Employer Identification Number)
9. Questionnaires (only as necessary):
